Just tested that plugin with the theme and it is working fine

Thank you for letting us know and providing details of the plugin that is causing the issue, I am sure this will help other members too.
On our installation Add media button is working fine in WordPress 4.0.
To Troubleshoot:
– Check after disabling all plugins
– Switch to default theme and then check the Add media button, revert and check again, after this you will know if it’s a theme issue.To remove display of category and tags on home refer:

To remove author name: Go to Control Panel -> Main Settings -> Turn OFF Author on Home page.
That area seems to be generated by some third party plugin, please contact the plugin author, he may be able to help you out.
Use FTP software like filezilla to delete the theme and start again by downloading fresh from member area or replace the functions.php file from the new download.
Another easy way is to ask your host to restore your website to previous day. Try this first.
Could you let me know what were you trying to do inside functions.php?
Dump the old cache from your caching plugin, refer previous reply.
